Many youth sports leagues and tournaments required teams to collect player physicals and other medical documents. NSID helps organizations collect these documents and makes it a part of the normal process of a player joining a team and getting verified. Below you can learn how a parent can upload a physical, medical document or another document that is requested by a league or tournament.
Instructions:
- Join your child to his or her team.
- With each NSID page load the Roster Data Form will pop up.
- Scroll to the bottom of the form and click Select Document.
- Browse your computer and upload the document(s).
NOTE: YOUR CHILD MUST BE JOINED TO HIS OR HER TEAM IN ORDER FOR THE ROSTER DATA FORM TO POP UP. How to Find and Join a Team
NOTE: You can upload multiple documents at once as long as the files are JPG’s. They will then be converted into PDF’s and a multiple page document.
